What is a Release form for?
A release form is an agreement between you and LLLGB, under which you give us your permission to use your image, story or other materials you have contributed towards a project, in our print publications, social media, website and other end products such as films. We also offer you the option of giving permission for La Leche League International to use your image in their work, in which case LLLI and any of the 70 countries where LLL is active may use it.
Please read this document as, once it is signed by you, it is a legally binding document between you and us.
Why do we need a Release form?
To make sure you understand and agree that your image, story or other work may appear in LLLGB’s general publicity material or as part of a named project. It offers you and LLLGB protection against your contribution being used in a way you have not agreed to.
What happens if I sign this Release form?
You allow LLLGB to use your image, story or any other materials you have contributed for such purposes as we may decide. LLLGB will only ever knowingly use an image or a piece of work to promote our mission and to give positive messages about breastfeeding and mothering. By signing this Release Form you understand that you will be giving LLLGB permission to decide how to use your contribution without future contact with you.
You can chose if you would like to be credited for your image or words, or if you would like to remain anonymous. When we refer to “images” this includes still pictures and videos.
What happens if I don’t sign?
We won’t use your image or work.
Who can sign?
If you are over 13 years old you can sign the form yourself. We need the signature of a parent, guardian or carer to sign on behalf of anyone under 13.

For all images – please check with the person who took the photo that they are happy for us to use it: if was a professional photographer they might hold the copyright to the image.
